The Value of Working with an Aging Life Care Professional
Our Care Team is accessible to clients, caregivers, families, and collaborative clinicians. Consultations are completed by a Geriatric Care Manager, Board Certified Nurse Practitioner, Certified Hospice & Palliative Nurse, ELNEC Award of Excellence Winner, Nurse Educator with Veterans Affairs Care Manager experience. Our team includes a licensed clinical social worker with expertise in emergency care, veterans affairs medical care and home-based primary care services. Our network is linked with elder professionals, healthcare agencies, attorneys, and community services within the service area.
Our Clients are chronically and seriously ill patients. They are considered vulnerable and high risk for hospitalization or acute change in condition. Chronic care is two or more conditions that are expected more than three months. Seriously ill patients may have multiple comorbidities with more than 50% include depression, anxiety, and chronic pain. More than 80% of these patients defer their care needs or advance care planning secondary to psychosocial barriers.
