​Long Life Care Management takes caregiver support seriously.  We provide timely, individualized consultation along with valuable collaboration.  Our chronic, transitional, and geriatric care management enhance quality of life goals and client satisfaction. This includes years of experience with both out-patient and in-patient care.  We are Aging Life Care™ Managers qualified and licensed professionals.

Our Standards of Practice and Code of Ethics



Our Care Team is accessible to clients, caregivers, families, and collaborative clinicians.  Consultations are completed by a Geriatric Care Manager, Board Certified Nurse Practitioner, Certified Hospice & Palliative Nurse, Nurse Educator with Veterans Affairs Care Manager experience.  The team includes other advanced degree professionals, such as healthcare providers, attorneys, and community members that reside within the service area.   

Our Clients are chronically and seriously ill patients.  They are considered vulnerable and high risk for hospitalization or acute change in condition.  Chronic care is two or more conditions that are expected more than three months.  Seriously ill patients may have multiple comorbidities with more than 50% include depression, anxiety, and chronic pain.  More than 80% of these patients defer their care needs or advance care planning secondary to psychosocial barriers.
